At Oak Pavilion, our care plans are created to address the medically complex needs of
our residents, while promoting the highest possible quality of life and levels of function.
Our professional staff, led by our Medical Director, consists of attending physicians,
physical, occupational, speech therapists, nursing staff, activities director, care plan
coordinators and social workers. Specialty medical services such as X-ray, dental and
podiatry care are provided on-site and transportation to medical appointments can be
arranged. We offer a full suite of services and amenities to meet resident needs including:

The ultimate goal of our rehabilitation services is to help individuals return to their
homes following orthopedic surgery, an accident or a medical event such as heart attack
or stroke. We offer an interdisciplinary approach which includes physical, occupational
and speech therapies. When the time is right, rehabilitation services transition
to restorative services as we test the durability of newly acquired skills and help patients
continue to improve their strength and stamina. We strive to involve family and at-home
caregivers to prepare them to support the patient after discharge. If necessary, we can
also make in-home visits prior to discharge to identify and remediate any barriers within
the environment that might present challenges to a person’s ability to fully function
independently within the home setting.
   
Short Stay Options
   
Oak Pavilion offers short stay options to meet the needs of today’s caregivers and family
members. Whether it’s for a weekend, a few days or several weeks, Oak Pavilion’s
respite care services offer the solution for caregivers in need of a well deserved break, a
vacation or other situation that requires short-term, full-time care. Our Admissions
Director can provide you with further information on this program.
